Background Investigation


Instructions to Candidates

NYS Courts Applicant Verification UnitNYS Court Officer-Trainee candidates who pass both the Physical Ability Test and Vision Screening Test must follow the instructions below and bring the following documents to their scheduled Background Interview.

1. Download and Complete:

  1. PERSONAL HISTORY QUESTIONNAIRE BOOKLET (UF-1)
    Complete the booklet in its entirety. EVERY QUESTION MUST BE ANSWERED (you may indicate, “does not apply,” only when applicable). Answer every question with truthfulness and without evasion.
  2. AUTHORIZATION FORM (UF-3)
  3. NYC DEPT OF SOCIAL SERVICES FORM
  4. NYS DEPT OF LABOR FORM
  5. US MILITARY RECORD REQUEST
  6. PHOTO REQUIREMENTS

2. Change of Personal Information

  1. CHANGE OF INFORMATION FORM (UF-11a)
This form may be used during the screening process to update a change of name, marital status, address, phone number and/or e-mail. Please return the form to: avu@nycourts.gov or fax to (212) 428-2778.

3. Detailed Income Earnings Printout Form (SSA-7050) FROM THE SOCIAL SECURITY ADMINISTRATION

  1. The printout must be submitted at your scheduled Background Interview.
  2. The printout must cover the period from the year you left High School to present.
  3. You must obtain the printout by visiting a SOCIAL SECURITY ADMINISTRATION office.
  4. It may be obtained on the same day ONLY if requested in-person.
  5. Do not request by mail as it may take up to 6 to 8 weeks for you to receive it, resulting in a delay of your investigation.
