INSTRUCTIONS TO COMPLETE THE E-Z 
SMALL CLAIM/COMMERCIAL CLAIM APPLICATION
There are four types of claims which can be filed:
  • Small claim - Individual suing individual or company.
      Required forms - Application form only
  • Commercial claim - Company suing company or individual.
      Required forms - Application  form, Certificate of Authority and Certification on Filing Limits
  • Consumer Transacation - Company suing individual in a Consumer Transaction case.
      Required forms - Application form, Certificate of Authority, Certification on Filing Limits and Certification of Demand Letter sent
  • Counterclaim - Individual or company named as a defendant who wishes to file a claim against the claimant individual or company.
      Required forms - Application form only

 
 
DIRECTIONS:

Please read this entire page before proceeding to the E-Z application form.

Complete the Application form first.  When completing the application, you must select the type of claim by clicking on and filling in the corresponding circle for the type of claim you are filing (see types of claims above).  You may then proceed to complete each field using your keyboard and advancing to the next field by using the "tab" key or moving the cursor with your mouse.  After all fields are completed,  print the application form using your browser print button.

If additional  forms are required (based on the type of claim selected - see above), you will automatically be referred to the "next" form when you click on the "next" button.  Complete each form as appropriate (using your keyboard and completing all required fields) and print using your browser print button before proceeding to the "next" form.
Select "Next" to begin application process.
