ORDERED that its decision and order on motion dated April 12, 2007, in the above-entitled matter is amended by adding to the second decretal paragraph after the word "granted" the following: "and on or before May 11, 2007, the appellant shall file serve and file an appendix containing a copy of Blank Official Financial Disclosure Affidavit, Family Court Official Form 4-17".
