ORDERED that the motion for permission to proceed as a poor person and to proceed upon the original file of the Workers' Compensation Board is granted, without costs, only to the extent that the filing fee required by CPLR 8022 is waived, and the motion in all other respects is denied, and it is further

ORDERED that the motion for an extension of time to perfect the appeal is denied, without costs, and the appeal is dismissed.
