| DECISION & ORDER ON APPLICATION |
Application by Tessa J. DiDomenico, who was admitted to the Bar at a term of the Appellate Division of the Supreme Court in the Second Judicial Department on February 11, 2004, to change her name on the roll of attorneys and counselors-at-law to Tessa Jean Forte.
Upon the papers submitted in support of the application, it is
ORDERED that the application is granted; and it is further,
ORDERED that the Clerk of the court shall change the applicant's name on the roll of attorneys and counselors-at-law from Tessa J. DiDomenico to Tessa Jean Forte, effective immediately.
