On the court's own motion, it is
ORDERED that its decision and order on motion dated December 3, 2008, in the above-entitled matter, which granted a motion by the appellant's assigned counsel to be relieved of the assignment to prosecute an appeal from an order of the Supreme Court, Kings County (IDV part), dated May 20, 2008, is amended by deleting the address for Kenneth M. Tuccillo, Esq., "251 E. 61st Street, New York, New York 10021," and substituting therefor the address "14 Edgewood Avenue, Hastings on Hudson, New York 10706."
