ORDERED that the decision and order on motion of this Court dated November 21, 2014, in the above-entitled matter, on an appeal from a judgment of the Supreme Court, Queens County, rendered April 21, 2009, is amended by deleting from the preamble thereof the date "July 23, 2014," and substituting therefor the date "September 17, 2014."
