ORDERED that the decision and order on motion of this Court dated April 23, 2015, in the above-entitled matter, is amended by deleting from the eighth decretal paragraph thereof the address "303 East 57th Street, New York, N.Y. 10022," and substituting therefor the address "31 Elizabeth Lane, P.O. Box 5025, Quogue, N.Y. 11959."
