Appellant having moved for permission to proceed as a poor person on the appeals taken herein from orders of the Family Court, Chautauqua County, entered in the Office of the Clerk of said Court on June 24, 2014, and October 24, 2014, and for other relief,
Now, upon reading and filing the affidavits of Sunshine J. Carroll, sworn to March 31, 2015, the affirmations of David J. Pajak, Esq., dated April 20, 2015, and April 30, 2015, the notice of motion with proof of service thereof, the affirmation of Andrew F. Emborsky, Esq., dated April 23, 2015, and the affirmation of Lydia V. Evans, Esq., dated April 28, 2015, and due deliberation having been had thereon,
It is hereby ORDERED that the dismissal of the appeal from the order of Family Court entered June 24, 2014, is vacated, and appellant shall perfect the appeal on or before July 6, 2015, and in the event of failure to so perfect the appeal is hereby dismissed without further order, and
It is further ORDERED that the motion insofar as it seeks permission to proceed as a poor person is granted, and
It is further ORDERED that the Clerk of the Family Court, Chautauqua County, is hereby directed to provide appellant with copies of all papers and transcripts of stenographic, digitally recorded, or audiotaped minutes, if any, of the proceedings had and filed in said Clerk's Office upon which the appeals are based.
