ORDERED that the decision and order on motion of this Court dated September 17, 2015, in the above-entitled matter, on an appeal from a judgment of the Supreme Court, Queens County, rendered October 26, 2011, is amended by deleting from the fifth decretal paragraph thereof the date "October 31, 2011," and substituting therefor the date "October 13, 2011."
