ORDERED that the decision and order on motion of this Court dated July 30, 2015, is amended by deleting from the second decretal paragraph thereof the address "5 Waller Ave - 2nd Floor, White Plains, New York 10601" and substituting therefor the address "150 Grand Street, Suite 305, White Plains, New York, 10601."
