| DECISION & ORDER ON MOTION |
Appeal by Stefanie Veras from an order of the Supreme Court, Kings County, dated March 11, 2016. By decision and order on motion of this Court dated June 28, 2016, issued pursuant to § 670.4(a)(2) of the rules of this Court (22 NYCRR 670.4[a][2]), the appellant was directed, inter alia, to file in the office of the Clerk of this Court an affidavit or affirmation containing certain information, on or before July 28, 2016. The appellant has failed to comply with the decision and order on motion of this Court dated June 28, 2016.
Now, on the Court's own motion, it is
ORDERED that the appeal is dismissed, without costs or disbursements, for failure to comply with the decision and order on motion of this Court dated June 28, 2016 (see 22 NYCRR 670.4[a][2],[4]).
