ECISION & ORDER ON APPLICATION

Application by Samantha Addonizio-Jacobellis, who was admitted to the Bar at a term of the Appellate Division of the Supreme Court in the Second Judicial Department on May 17, 1995, to change her name on the roll of attorneys and counselors-at-law to Samantha Addonizio-Butts.

Upon the papers filed in support of the application, it is

ORDERED that the application is granted; and it is further,

ORDERED that the Clerk of the Court shall change the applicant's name on the roll of attorneys and counselors-at-law from Samantha Addonizio-Jacobellis to Samantha Addonizio-Butts, effective immediately.
