ORDERED that the decision and order on motion of this Court dated February 14, 2017, in the above-entitled matter is amended by deleting from the eighth decretal paragraph thereof the addresss "125-01 Queens Blvd., Kew Gardens, NY 11415" and substituting therefor the address "110-11 Queens Boulevard, Apt. 10H, Forest Hills, NY 11375."
