ORDERED that the order on application of this Court entitled "[i]n the Matter of Applications for Extensions of Time," dated September 20, 2018, is amended by deleting from the "Applicant Name" entry concerning the above-entitled matter the name "Maimonides Medical Center" and substituting therefor the names "Victor Fariwa and Avenue U Medical Care, P.C."
