| ECISION & ORDER ON APPLICATION |
Application by Donna Ann Civitello, who was admitted to the Bar at a term of the Appellate Division of the Supreme Court in the Second Judicial Department on April 25, 1984, under the name Donna Ann Ciampa, to change her name on the roll of attorneys and counselors-at-law to Donna A. Ciampa.
Upon the papers filed in support of the application, it is
ORDERED that the application is granted; and it is further,
ORDERED that the Clerk of the Court shall change the applicant's name on the roll of attorneys and counselors-at-law from Donna Ann Civitello to Donna A. Ciampa, effective immediately.
