ORDERED that the decision and order on motion of this Court dated September 23, 2019 in the above-entitled matter, on an appeal from a judgment of the Supreme Court, Nassau County, rendered May 9, 2018, is amended by deleting from the Appellant's Address thereof the words "East Elmhurst, NY 11554" and substituting therefor the words "100 Carman Ave., East Meadow, NY 11554."
