Initial Application and Filing

  • Start by filling out the Application for a Temporary ERPO (UCS‑6341), available as a fillable or plain PDF.
  • File it together with a Request for Judicial Intervention (RJI, UCS‑840).
  • Bring both forms to the Supreme Court in the county where the respondent lives.

Court Review and Hearings

  • The page explains what happens at the courthouse, how a judge reviews your filing, and how a hearing is scheduled.

After a Final ERPO

  • The page includes additional forms used after a final ERPO is issued.
  • The respondent may file one request to change or cancel the ERPO (UCS‑6343V).
  • The petitioner may request a renewal (UCS‑6345A) within 60 days before the order expires.
  • Separate forms exist for returning firearms: – To the lawful owner (UCS‑6343A) if the respondent is not the owner.
    • To the respondent (UCS‑6346A) after the ERPO expires and is not renewed.
