The New York State Unified Court System's RJI Forms page provides official Request for Judicial Intervention (RJI) forms used in Supreme Court civil cases to formally request that a judge be assigned to a matter.
The page offers the main RJI form (UCS-840) along with several case-specific addenda, including forms for General, Commercial Division, Foreclosure, Matrimonial, and Partition matters, as well as a separate Uncontested Matrimonial RJI (UD-13).
