The Fee Waiver Application page provides forms for individuals who are unable to afford court costs, fees, and expenses in New York State courts.

The page includes four forms available in both fillable and plain PDF formats:

  • Application to Waive Court Costs, Fees, and Expenses (UCS-FW1) to initiate the request
  • Notice of Motion to Waive Court Costs, Fees, and Expenses (UCS-FW2)
  • Affirmation of Service confirming the motion was properly served (UCS-FW3)
  • Order granting the fee waiver (UCS-FWO1) for the court's use
