Instructions for Completing Form Fields

  1. Field 1: Insert the county where the action was brought.
  2. Field 2: Print the index number assigned to the case.
  3. Field 3: Print the Plaintiff's name.
  4. Field 4: Print the Defendant's name.
  5. Field 5: Insert the county where the action was brought.
  6. Field 6: Print the date the judgment of divorce was entered. This date appears in the entry stamp on the judgment. If there is no stamp on the document, bring it to the County Clerk's Office to have it entered.
  7. Field 7: List the date this document was prepared.
  8. Field 8: List the preparer's name.
  9. Field 9: Write the preparer's address.
  10. Field 10: Write the Defendant's name and address, or, if the Defendant has an attorney, the attorney's name and address, and check the appropriate box. This Notice of Entry will be sent with the Judgment of Divorce to the Defendant, as described on page 12 of the instructions booklet under the heading “After the Papers are Approved.”
