Retired Identification Cards 

These cards are issued to Judicial personnel upon their retirement from the NYS Unified Court System (UCS). (effective 3/1/2026)


Judges / Justices upon retirement:

  1. Judicial personnel who wish to receive a retiree identification card from OCA must return their OCA issued employee ID card.
  2. Retired ID Form Requests must be submitted to the Chief Administrative Judge’s office, within 60 days of retirement, and have that person approve the application form (see form attached). Upon approval, the form will be submitted to Department of Public Safety.


The Department of Public Safety will issue a retired identification card once it has received the employee’s identification card and request form confirming the requestor’s status as retired in good standing. 
Former employees who retired in good standing after December 31, 2025 and who have lost their old retiree identification card, may request a replacement retiree Identification card for a $25 fee (money order payable to the NYS Office of Court Administration. No other form of payment will be accepted.) The requestor must request an application form from their former command, complete the form, and attach their money order. At that point, all materials should be mailed or submitted in person (appointment only), by the requestor, to:

Instances where the requestor’s photograph is not currently on record, the requestor must make an appointment with the Department of Public Safety to arrange for a photograph to be taken. The Department of Public Safety will issue a replacement retiree identification card once it has received the $25 fee, completed application, and police report (if lost/stolen) or damaged ID. The Department of Public Safety will send the retiree identification card, by certified mail, to the requestor’s address as maintained in the court system’s Human Resource’s Information System. In the event that the requestor wants the retiree identification card sent to an alternative address, the application must include the new address in the allotted space along with the requestor’s signature (See Form).
